Health Insurance Portability and Accountability ACT
The Irving ISD Benefits Office may sometimes need to
disclose medical information or payment information protected by the
Health Insurance Portability and Accountability Act in relation to our
group health plans to your family members or close friends involved in
your health care.
The Notice of Privacy Practices followed
by Irving ISD with regard to your Protected Health Information follows.
A form for the purpose of designating with who we may
discuss your medical or payment information is also included following
the Notice. When needed, the form may be printed from the website
and faxed to the Benefits Office at 972.215.5239.
