If you are planning to take a leave of absence for more than 5 consecutive days, you must complete an Application for Lengthy Leave.

Here is an Overview of the Family Medical Leave Act (FMLA).  If you qualify for FMLA leave, your position is protected for up to 12 working weeks with medical documentation. The law does not provide income during the leave, however, if you do have local, state or personal leave days available, they may be used concurrently with the FMLA leave so that you can be paid for the days available to you.  Review the law to see if you are eligible for FMLA leave.

Review DEC Local Board Policy to review leaves available to you.   

A completed Certification of Health Care Provider is required when requesting Lengthy Leave.  The form must be turned in to the Benefits Office.

If you are a member of the Sick Leave Bank, you may apply for leave from that source if all other leaves will be exhausted during your lengthy leave.  Complete the packet in its entirety.
