If you are planning to take a leave of absence for more than 5 consecutive days, you must complete an Application for Lengthy Leave.

It is recommended that you include a completed Certification of Health Care Provider (below) when submitting your Application for Leave when possible.”

- Certification of Health Care Provider for Family Member's Serious Health Condition

A completed Certification of Health Care Provider is required when requesting Lengthy Leave.  The form must be turned in to the Benefits Office.

The eligibility notice will be completed by HR once application for leave is received or if HR is notified of an absence that may qualify for FMLA.

The district will request confirmation of delivery and estimated return to work date within 2 weeks of delivery.

This form must be returned to the Benefits Office one week prior to return to work.  No employee will be allowed back to work following an extended absence without doctor's release.

Family Medical Leave Act Update

The Family Medical Leave Act has been updated.  The changes are effective January 16, 2009.  The changes in the law deal with notification timeframes, eligibility, new medical certification requirements and providing leave for families of certain military personnel.  Please see the attached document to understand how these rules effect you.

Here is an Overview of the Family Medical Leave Act (FMLA).  If you qualify for FMLA leave, your position is protected for up to 12 working weeks with medical documentation. The law does not provide income during the leave, however, if you do have local, state or personal leave days available, they may be used concurrently with the FMLA leave so that you can be paid for the days available to you.  Review the law to see if you are eligible for FMLA leave.
