    Enrollment Form for Life Insurance

The District has selected CIGNA as the new provider for Term Life and Voluntary AD&D Life coverage effective 9-1-2011.  With the change all benefit eligible employees have the opportunity to sign up for up to 2 times their annual salary (not to exceed $200,000) with no questions asked.  You are automatically approved!   This is a one time opportunity for our employees.  You will need to make your election on the In-Roll system.

Along with your life insurance CIGNA is also providing these programs at no additional charge:

Your monthly cost of insurance will depend on your age and the amount of insurance you wish to purchase.  Note that at age 65 your benefits are reduced.
