Nomination Form
to Serve on the DATE Committee

District Awards for Teacher Excellence (DATE)

If you wish to nominate yourself for the DATE Committee, please complete this form and give to your principal.  The DATE Committee is charged with creating a district incentive plan for teachers and other employees.  It will be similar in scope and intent to the Texas Educator Excellence Grant (TEEG), but on the District level rather than the campus level.  This committee will likely meet once or twice a month during the 2007-2008 school year, with two hour meetings beginning at 4:30 PM.

The principal will present your nomination to the campus CIC, who will approve the nomination to the District Improvement Committee (DIC) Chairperson.  The DIC Chairperson will select the final Committee from the CIC nominees.

Below you will find the complete list of positions on the DATE Committee.   For more information on the DATE program, go to the following website:  http://www.tea.state.tx.us/ed_init/eeg/datex/

Please turn in completed form to the campus principal.

(October 15th deadline.)
