Dental ID Cards  

You should have received your Dental ID card already if you enrolled in a CompBenefits Dental plan.  You will only receive 1 ID card for the Dental plan regardless of the number of covered dependents.  You will not receive an ID card for the Vision plan.

If you have not received your dental card you can print one off directly from www.compbenefits.com.

Vision Benefits

You will not receive an ID card for the vision plan.  To use the plan:  call and make an appointment at any participating Visioncare provider.  Let them know you are on Visioncare and the doctor’s office does the rest.  To located a provider call Member Services 1-800-865-3676 or visit  www.visioncare.com
