- Bring the following to your appointment to verify eligibility:
- Seniors (55+)
- Driver License
- Military
- Driver License AND
- Veteran Health Identification card
- Proof of Service letter
- DD-214 Form
- Seniors (55+)
- Receive your Gold Card
