Exemption from Immunization Requirement
Chapter §97.62 of the Texas Administrative Code (TAC) describes the conditions under which individuals can seek exemptions from Texas immunization requirements. Exclusions from compliance are allowable on an individual basis for medical contraindications, reasons of conscience, including a religious belief, and active duty with the armed forces of the United States.
Texas Conscientious Exemption
Texas law allows parents or guardians to claim a conscientious exemption from required immunizations for school attendance. A student may not be enrolled until the school receives the original, notarized Conscientious Exemption affidavit. To obtain this affidavit, request the form from the Texas Department of State Health Services (DSHS), download, print it, check boxes of immunization that the student is to be exempted from, have it notarized, and submit the original document to the school nurse at the time of registration.
