To request that the school nurse or a trained staff member perform a healthcare procedure for your child during the school day, please complete the form linked below. All procedures are carried out only with written physician orders and in accordance with the provider’s instructions to ensure student safety and appropriate care.
Parents or guardians are responsible for providing all required equipment, supplies, medications, and related items needed for the procedure. These items must be maintained in good working order and replaced as needed throughout the school year. Please contact your school nurse with any questions or to discuss your child’s specific healthcare needs.
