If your child needs special food accommodations due to a disability, allergy, or risk of anaphylaxis, please complete the Special Diet form on this page.
This form is required for students with dietary needs related to a medical condition or disability. It should be updated as needed—ideally at the beginning of each school year.
A licensed medical doctor or other approved healthcare provider must fill out the relevant sections of the form.
