Americans with Disabilities Act (ADA)

 

Applicants, candidates or employees may request an ADA accommodation through Risk Management Department.

To submit a request, please complete this form. [Please note that you will be required to submit documentation from your healthcare provider, outlining the diagnosis and limitation of the employee and the accommodation(s) required.] We reserve the right to request additional or clarifying medical information using our own forms if necessary to evaluate the accommodation need. 

An accommodations review committee (ARC) may be scheduled to review the request. The Director of Benefits and Risk Management will notify the employee and immediate supervisor of the decision and implementation instructions, if applicable.
