Pre-K Registration 2008-2009 Who : Children who will be 4 years old on or before September 1, 2008 When : July 30, 8:00 - 6:30 Where : Austin Middle School Bring the following items in order to register : * Current Immunization Record * Social Security Card * Social Security Card *Official Birth Certificate * Proof of Residence - apartment lease, gas bill, water bill, or electric bill, if residence is not your own a notarized letter from primary resident / home owner is required for proof or residency * Proof of Income - current paycheck stub, current pay envelope, letter from employer stating gross wages paid and how often they are paid * Proof of Military dependent (if used for eligibility) * Proof of conservatorship if child has been in foster care with the Department of Family & Protective Services.
